Aged 33, Miner, he was working in a wall 14 feet wide along with another miner, and when in the act of filling a waggon was killed by a fall of dogger and sulphur; along the centre of the working there was a slight dislocation of 10 inches and the roof was entirely unsupported for a distance of 17 feet back from the face; a shot had been fired in the bottom stone only 20 minutes before and the roof had not been examined afterwards; the roof was damp and sufficient care was not exercised examining it, and timber should have been set nearer the face as the presence of the fault was well known
